Medical Information/Emergency Forms

 

Medical Information forms completed at the start of the school year are on file in the Health Office. These forms include parent’s home and work phone numbers, persons to contact if parents cannot be reached, and doctors' names with contact numbers.
 

It is vitally important that you keep the Main Office/Health Office apprised of any changes in address, telephone numbers, places of employment, and persons to be contacted in case of emergency.
 

In medical emergencies, schools may only give first aid and must be able to contact someone who can make vital decisions about medical care. Treating a child in the hospital REQUIRES parental consent.

A phone call or note from you helps the Health Office keep up to date information to meet the unexpected needs of your child.
