MEDICATIONS

 

****No Medication of any Kind Should Be Sent to School With The Student, including prescription as well as over the counter medications.

 

According to New York State Law, the following procedure must be followed in order for a school nurse to administer medication to a student:
 

1.   The parent or parent’s adult designee must deliver the medication, in its original pharmacy labeled container, directly to the School Nurse.

2.   The School Nurse must have written permission from the physician, which indicates the name of the student, the diagnosis, and the dose and frequency of the medication. The prescription bottle label is NOT sufficient.

3.   The School Nurse must also have written permission from the parent/guardian to administer the medication as directed by the physician.

 

When this procedure is not followed, the parent/guardian will be called to come to school to administer the medication. If this is not possible, the medication will not be given
