Sports Physicals

All students participating in interscholastic sports are required to have a physical exam either by their own physician or the School Health Service. If a private physician performs the physical exam, the school physician/nurse practitioner must still authorize the student to participate in a sport.
An Athletic Health History Form, completed by the parent/guardian is also
required to be filed with the School's Nurse.
Both the physical exam and the Health History Form must be submitted, and then approved, by the School Health Service before a student can participate in “tryouts” or practices.
The physical exam is valid for 12 months. However, prior to each sports season the health history form must be updated by the parent and submitted to the School Health Service for review and approval.
The school offers “Sports Physicals” in June for the
following school year. This is especially important for those students
interested in participating Fall Sports (soccer, track, cross country). Having the physical
completed in June by either the school Nurse Practitioner or a private
physician insures that all the necessary paperwork will be on file before the
start of the next school year. This will avoid any delay in participation in the
fall.
